Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce the case of a corrective surgery to correct a patient’s short and upturned nose.
Since this patient had facial asymmetry, this case required a delicate approach to determine the overall facial harmony together with the nasal correction.
Preoperative Design Consultation
- An overall wide and short nose
- Excessively visible and asymmetric nostrils
- Plain and deviated nose caused by the facial asymmetry
- Misalignement of the center of the glabella, nose tip, philtrum and chin.
- Upturned nose tip
- Big nasolabial angle and protruding mouth
- Prominent retracted chin
- Flat glabella and short nose
- More visible protruding mouth
- Blunt and upturned nose tip
- The right nostril is smaller than the left
- Columella crushed to the right side
- Visible nostril asymmetry
Surgical details
The following surgeries were performed using a closed rhinoplasty approach:
a) Rib cartilage harvesting and transplant:
- Reinforcement of the cartilage on both sides of the nose tip for the surgery using autologous rib cartilage from the patient.
b) Septal extension graft surgery
- Strenghtening and extension of the septal cartilage using the rib cartilage.
- This corrects the short lenght of the nose and lowers the upturned part of it.
c) Septal reconstruction surgery:
- Moving the septal cartilage that was crushed on the right side to the left side and fixing it in order to improve the nasal asymmetry.
d) Adjustment of the nose tip location:
- To enhance the facial asymmetry, we move the nose tip to the left side.
e) Insertion of an implant on the nose bridge:
- The implant was positioned approximately 1 mm to the left of the center of the face to minimize facial asymmetry.
f) Columella correction
- Lowering of the retracted columella using the rib cartilage.
- This gives an improved effect to the protruding mouth.
g) Improvement of the nostril asymmetry
- Great natural improvement of the nostril asymmetry by moving the columella to the left side.
Before and After surgery photos
- The upturned nose and visible nostrils appearance disappeared naturally.
- The wide and plain nose became more three-dimensional and slimmer.
- Straightening of the deviated nose.
- Improvement of the overall facial balance as the asymmetry was improved.
- The upturned nose tip was naturally lowered, and the overall shape improved.
- Proportion enhancement as the short nose became longer
- Nasolabial angle correction improved the appearance of the mouth protrusion.
- The upturned and blunt nose tip became sophisticated and more natural-looking
- Improvement of the overall balance of the face by the correction of the protruding mouth
- The columella that was pressed on to the right side was corrected and fixed at the center.

This case is one that simultaneously addresses the complex issues of a short and upturned nose, and facial asymmetry. In particular, in cases of facial asymmetry, it is necessary to take on a careful approach that considers the overall facial balance, rather than simply correcting the nose.
Septal extension graft surgery using autologous costal cartilage was effective in correcting a short and upturned nose. In addition, we were able to solve the nostril asymmetry issue by adjusting the position of the columella through septal reconstruction surgery.
The key to this surgery was to finely adjust the position of the nasal tip and bridge implants to minimize the facial asymmetry. This allowed for not only nose correction but also improvement in overall facial balance.
All procedures were performed using a closed approach, completing the surgery without external scars.
