1. Peoperative consultation
The patient visited my clinic because she wanted to correct the feeling that the tip of his nose was uneven and asymmetrical, and that the tip of his nose looked distorted as a whole. The columella was retracted and the starting point of the nose was high, creating a strong impression as eyes were narrow.
2. A surgical plan
Considering the possibility of mild contracted nose due to the shape and the hardness of the nose tip, I planned to make the tip natural using the self-rib cartilage. And Considering the bumpy shape of the nose tip, I expected that the damage to the alar cartilage could be severe. I decided to remove the silicon implant from the nose bridge and insert a new one lowering the starting point. I planned to correct the nasolabial depression to create a natural shape of the nose tip and went into surgery.
3. Surgical findings
As expected, about 50% of the cartilage in the nose was cut off and did not exist, and the skin at the tip of the nose was very bumpy due to the severe adhesion to this area, and there were many hard tissues around the existing silicone implant, so it was judged that there was a chronic foreign body reaction.

5. A general review
The patient’ nose seemed to be contracted and she wanted a very natural nose. So the damaged cartilage at the tip of the nose was reconstructed using a self-rib cartilage. The curved silicone implant was removed, and the curved part was corrected by performing an external osteotomy. In addition, I made a natural nose that is not high by lowering the starting point between the eyebrows.
