Closed Rhinoplasty Korea + Long and Droopy Nose + Nose Bump

1. Preoperative Consultation

This patient visited my clinic to fix a long nose with a bump. This person had originally a high nose bridge, and previously went to another hospital to fix the nose bump, however, the nose bump remained and the droopy nose tip and long appearance was not corrected at all.

2. Surgery Plan

In order to correct the long and hooked nose, I planned to cut the bump through an outer osteotomy and to gather the droopy bridge of the nose. The plan for the wide parts of the nose tip was to sturdily lift the nose tip with the self-rib cartilage and reduce the droopy columella. Since the skin of the nose tip is thin, the plan was to transplant the nose tip cartilage, and use artificial dermis to reinforce the skin in the visible parts of the cartilage.

3. Surgical Opinion

To correct a long nose, the nasal bone needs to be cut and the nose tip needs to be raised slightly. To correct the hooked nose, the nasal bone and the upper cartilage were removed partially, and in order to cover the bumpy parts, I inserted a thin silicone implant. The bent parts of the columella were corrected.

5. General Overview

This was a long and hooked nose surgery. In many cases, a nose bump severely elongates the nose. By fixing the nose bump, the droopy appearance of the nose tip can be fixed, and the length of the nose can be improved to look more natural. The nose bump consists of the nasal bone and the upper cartilage, once the nasal bone part is trimmed through an osteotomy, it can be corrected. Since the upper cartilage is not cut, it must be cut directly. Moreover, if the nasal bone is cut, it can become thin, resulting in deformation that opens the roof of the nasal bone, therefore an outer osteotomy is performed to solve this issue. A hooked nose needs to be slightly over corrected to make it look neat.
