Closed Rhinoplasty Korea + Crooked nose

1. Preoperative Consultation

The patient came to the clinic to correct a crooked nose and high nose starting point. The silicone implant was bent to the right, and the nose starting point was so high it seemed too close to the eyes. This gave the upper area of the face a harsh appearance. The asymmetry of the nose was emphasized since the nostrils were visible from the front.

From the front, the nose implant is bent to the right, and the nostrils are visible, emphasizing the asymmetry.

2. Surgery Plan

The plan to improve the patient’s nasal congestion was to perform a septoturbinoplasty. The silicone implant would be removed, the nasal bone asymmetry would be corrected using an lateral osteotomy, the dorsum would be trimmed, and, the new implant would be placed at the center with a lower nose starting point. The nose tip was planned to be raised to the safest maximum height where the skin is able to be stretched using the patient’s own costal cartilage, and to be directed to a position where the nostrils are less visible. At the same time, we would implement a reduction of the bottom of the nose and the

The starting point of the nose bridge was lowered and the tip of the nose was advanced further than the lips in the side profile view.
The nasolabial angle was corrected, which improved the appearance of the protrusion.
In the 45-Angle View, it is possible to observe that the silicon prosthesis that was clearly visible has now a much softer appearance.
From the front, the deviation to the right has been corrected, and the size and shape of the nostrils were improved.
The width of the nasal base has also been reduced.

By looking at the photos of the nostrils, it is possible to tell that the area of the nose tip that was bent to the right, the nostril asymmetry,
and the width of the nasal base have improved significantly.

3. Overall review

This surgery was performed to correct a implant that was bent completely to one side. This problem can sometimes occur if a small nose is pushed too high. It is not easy to place a large implant in the center of a small and narrow nose with nasal bone asymmetry. First of all, it is important to select implant of the right height and to make sure that the bottom of the implant flattens the nose bone to some extent to avoid it from bending to one side over time.
