Today, I’m sharing a case involving a patient with a short, crooked nose and significant nostril exposure. We performed a revision rhinoplasty for short nose using autologous rib cartilage with minimal calcification to ensure long-term stability and a natural aesthetic result.

Patient History and Concerns

  • 15 years ago: Primary rhinoplasty using a silicone implant and ear cartilage graft

  • Crooked nasal bridge

  • Short nose and upturned tip

Preoperative Assessment

  • Chest X-ray confirmed calcification in several costal cartilage segments

  • Right 8th rib selected for its minimal calcification

 

Surgical Strategy

  • Structural reconstruction with autologous rib cartilage

  • Septal extension to lengthen the nose

  • Alar base lowering to reduce nostril exposure

  • Radix lowering to soften the profile

  • Tip refinement for a more balanced look

  • Simultaneous surgery to improve rhinitis symptoms

Strategic Cartilage Selection

By preoperatively identifying calcified costal cartilage, we were able to safely harvest a viable portion, minimizing risks such as pneumothorax and graft failure.

 

Multidimensional Correction

We addressed both aesthetic and functional issues—correcting the short nose, tip position, and nostril exposure while also improving breathing through turbinate reduction.
