Today, I’m sharing a case involving a patient with a short, crooked nose and significant nostril exposure. We performed a revision rhinoplasty for short nose using autologous rib cartilage with minimal calcification to ensure long-term stability and a natural aesthetic result.
Patient History and Concerns

Surgical History
-
15 years ago: Primary rhinoplasty using a silicone implant and ear cartilage graft
Main Concerns
-
Crooked nasal bridge
-
Short nose and upturned tip
-
Overexposed nostrils
-
Bulbous nasal tip
-
High radix (nasal starting point)
-
Chronic rhinitis symptoms

Surgical Plan for Revision Rhinoplasty for Short Nose
Preoperative Assessment
-
Chest X-ray confirmed calcification in several costal cartilage segments
-
Right 8th rib selected for its minimal calcification
Surgical Strategy
-
Structural reconstruction with autologous rib cartilage
-
Septal extension to lengthen the nose
-
Alar base lowering to reduce nostril exposure
-
Radix lowering to soften the profile
-
Tip refinement for a more balanced look
-
Simultaneous surgery to improve rhinitis symptoms
Results After Revision Rhinoplasty for Short Nose

Functional Improvements
-
Inferior turbinate reduction
-
Alleviation of rhinitis symptoms expected

Aesthetic Results
-
Straighter nasal bridge (frontal view)
-
Reduced nostril show
-
Appropriately lengthened and repositioned tip (side view)
-
Lower radix for a natural slope
-
Enhanced nasolabial angle
-
Improved nostril symmetry (nostril view)

Key Insights from This Revision Rhinoplasty for Short Nose

Strategic Cartilage Selection
By preoperatively identifying calcified costal cartilage, we were able to safely harvest a viable portion, minimizing risks such as pneumothorax and graft failure.
Multidimensional Correction
We addressed both aesthetic and functional issues—correcting the short nose, tip position, and nostril exposure while also improving breathing through turbinate reduction.
