Bulbous + Deviated + Upturned Nose Surgery – NOSELAB

Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today, we will introduce the corrective surgery case of a patient who had a deviated, bulbous and upturned nose at the same time.

This patient said that she wanted to fix her blunt nose and its upturned and deviated appearance to the right side, as well as increasing the height of the starting point.

Preoperative Design Consultation

Frontal view
  • Wide and blunt nose in general (bulbous nose)
  • Nose bent to the right (deviated nose)
  • Noticeable nostril asymmetry
  • Upturned nose tip
Side profile
  • High nose bridge starting point
  • Misadjusted nasolabial angle between the nose and the lips.
  • Unnatural upturned nose tip
45-degree angle view
  • The nasal deviation is more pronounced.
  • Characteristic bulbous and upturned nose.
Nostril photo
  • Visible nostril asymmetry due to the nasal deviation.
  • Blunt nose tip.

Surgical details

a) Autologous rib cartilage harvesting and transplant

  • Harvestingf the costal cartilage to secure a sufficient amount of cartilage needed for nasal reconstruction.

b) Septal extension surgery:

  • Strenghtening and extension of the septal cartilage using autologous costal cartilage.
  • This naturally lowers the tip of the nose and increases support.

c) Nose tip correction:

  • The blunt nose tip was transformed into a natural shape by removing the subcutaneous tissue, repositioning the alar cartilage, and tying the cartilage.
  • The height of the tip of the nose has been appropriately adjusted to improve the feeling of a bulbous nose.

d) Deviated septum correction:

  • The right-sided deviation of the septum was corrected to improve the tip of the nose, and the overall symmetry of the nose was improved.
  • Through this, nostril asymmetry was also naturally corrected.

e) Lowering the starting point :

  • The starting point of the nose bridge, which appeared high, was appropriately lowered to create a natural connection with the glabella.

f) Nasolabial angle correction:

  • Slight lowering of the columella to adjust the nasolabial angle appropriately.
  • Throught this, we were able to improve the artificial-looking upturned nose and transform it into a more natural-looking nose.

Every procedure was performed through the closed rhinoplasty approach, without leaving any external scars.

Frontal view before surgery (left), After surgery (right)
  • The nose, which used to look blunt and wide, has been improved to a slim and sophisticated shape.
  • Straightening the crooked nose.
  • Significant improvement of the nostril asymmetry
  • Lowering of the nose tip, improving the overall balance.
Side profile before surgery (left), After surgery (right)
  • Lowering of the nose bridge that was too high, in a manner that it is harmonious with the glabella.
  • Improvement of the relation of the lips and nose through the nasolabial angle adjustment.
  • The general proportions became more harmonious after lowering the upturned nose tip.
45-degree angle before surgery (left), After surgery (right)
  • The deviation correction made the nose line looks smoother.
  • The characteristic bulbous and upturned nose got a more natural and sophisticated appearance.
Nostrils before surgery (left), After surgery (right)
  • Great improvement of the nostril asymmetry that was caused by the deviation.
  • Improvement of the bluntness of the nose tip.

Overview – bulbous, deviated and upturned nose

This case is one that simultaneously solved the complex problems of a bulbous nose, a crooked nose, and a deviated nose. The patient’s nose shape before surgery was not severe, but it felt unnatural and artificial overall. Septal extension surgery using autologous costal cartilage played a major role in correcting the upturned nose tip and strengthening the overall structure of the nose. In addition, by straightening the deviated septum, we were able to solve the problem of nostril naturally. Lowering the starting point of the bridge of the nose and appropriately adjusting the nasolabial angle played a significant role in solving the problem of the nose looking artificial. This allowed us to create an overall natural and harmonious nose shape.
By using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving a scar, which greatly contributed to increasing patient satisfaction.
